You can fill it sign and fax to 0755992221 0r email to admin@solstice-mindmatters.com.au  If you have any security or privacy concerns please personally bring the form to your first appointment.

Reimbursements:

Appointments with psychologist may be eligible for reimbursement by Medicare if you have a referral from your GP with a Mental Health Care Plan.
Private Health funds may also cover psychological services.
